Description: Records relating to the installation, maintenance, operation, and use of telecommunications equipment and systems. Includes information on telephone, cable, fiber optics, electronic mail, voice mail, fax, pager, cell phone, and emergency systems and processes.

Documents include requests for installation or maintenance, change requests, copies of service contracts and maintenance agreements and copies of invoices.

Division: Health Information Services.

Retention and Disposal: 7 years after superseded, then destroyed.


Morgue Release and Storage Records   GCR/PIB

Type: General Class of Records/Personal Information Bank

Description: Records relating to the use of the morgue, release of bodies and storage of bodies.

Division: Health Information Services.

Types of personal information: Documents relating to identifying bodies in the morgue and may include personal information such as name, address, etc.

Record uses: Document morgue use and release of bodies.

Categories of users: Health Information Services Staff

Categories of individuals in bank: Deceased members of public.

Retention and Disposal: Indefinite.


Patient Records   PIB

Type: Personal Information Bank

Description: Records relating to individual patient care.

Division: Health Information Services.

Types of personal information: Patient personal information including such items as name, date of birth, address and medical information relating to that patient.

Record uses: To provide patient care.

Categories of users: Health Information Services Staff, medical staff and staff involved in the circle of care for patient services.

Categories of individuals in bank: Patients.

Retention and Disposal: 10 years post-discharge/death or 10 years after patient 18th birthday.

Legal Authority: Public Hospitals Act.


Patient Registration Documentation   GCR/PIB

Type: General Class of Records/Personal Information Bank

Description: Records relating to daily admissions and discharges, daily census summaries, admission requests from the emergency department and Ministry of Health log information.

Division: Health Information Services.

Types of personal information: Various patient-related information such as name, age, O.H.I.P. number, address, etc..

Record uses: Statistical reporting requirements and general Hospital planning.

Categories of users: Patient Registration staff and Managers of departments..

Categories of individuals in bank: Hospital patients.

Retention and Disposal: To be determined.
