Social Work/Discharge Planning
Social Workers provide a full range of services to patients and families connected with our in-patient and out-patient programs at Windsor Regional Hospital.
The Social Work Department participates in the provision of interdisciplinary health promotion and intervention, addressing the bio-social needs of the patient in accordance with the Mission Statement and Values of Windsor Regional Hospital.
Clinical services provided by Social Work include:
- adjustment to illness
- supportive counseling
- locating and arranging resources
- crisis/trauma counseling
- discharge planning
- placement options in consultation with hospital staff and community agencies
By acting as advocates and as liaison between patients and the community, social workers/discharge planners assist patients and families in obtaining financial aid, medication coverage or medical supplies through community based programs
Discharge planning is a service provided by Windsor Regional Hospital to assist patients and their families with the planning of a post-hospital care plan. With early preparation, Social Work can assist you and your family to make the transition back to the community as smooth and as non-stressful as possible.
