Now is the time you move on with your health and begin a new chapter. You are being discharged as your care with the Cancer Program is complete and your team is comfortable transitioning you to the next step. If you have a Primary Care Provider (Family Doctor or Nurse Practitioner), they are well-equipped to take care of you.
This package will help empower you to continue on with the rest of your life. We hope the information contained in this package is beneficial to you and your loved ones as you continue on your journey.
Additionally, Cancer Care Ontario provides Surveillance Guidance Summaries for survivors of the breast, colorectal, and prostate patient populations. These guidelines can be shared with patients at Discharge and are shared with Primary Care following discharge.
