Neonatal Developmental & High-Risk Growth and Nutrition Follow-Up Clinics
Purpose
These clinics provide ongoing care for infants discharged from the NICU who are at risk for developmental delays, growth concerns, or feeding/nutritional challenges.
 
Target Population - Infants and young children who:
     - Were born prematurely
- Experienced fetal growth restriction (FGR)
- 
     Required prolonged NICU stays or respiratory support
- 
     Have neurological or congenital conditions
- 
     Face feeding difficulties (nasogastric/ G-tube feeding) or growth faltering
 
Services Offered:
     - 
     Developmental milestone assessments using standardized testing tools such as Alberta Infant Motor Scales, Bayley Scales of Infant Development, Peabody Developmental Motor Scales
- 
     Growth monitoring and individualized nutrition plans
- 
     Feeding support (breast/bottle/supplemental)
- 
     Family education and psychosocial support
- 
     Coordination with community services and early intervention programs
 
Multidisciplinary Team Includes:
     - 
     Neonatologists
- 
     Occupational Therapists
- 
     Nursing
- 
     Dietitians
- 
     Social Workers
 
Referral Process:
     - 
     Automatic referral from NICU prior to discharge
- 
     Community pediatricians may refer
- 
     Scheduling based on clinical need and follow-up protocols

     Ensure continuity of care post-discharge
